14-year-old Omar wasn’t feeling his best, until his care team at OLE Health stepped in. OLE Health practices a new model of care that puts the patient in the center of a team of dedicated medical professionals, and Omar’s pediatric nurse practitioner and pediatrician both worked with him to implement a new focus on being healthy after determining he was experiencing prediabetes. Through referrals to behavioral health and a nutritionist, Omar – and his mother along with him – began following the plan outlined by his care team. Now he’s no longer prediabetic and has lost more than 30 pounds.
